| What to bring to a loan application |

At loan application, you will be asked for the following items:

- Name, age & social security number of borrower & co-borrower
- Accepted purchase agreement & copy of listing sheet
- Check for application fee (which will be applied toward appraisal fee & credit report fee) in the amount of $320.00 (Payable to Accessibility Mortgage, LLC)
- Income:
- Past 2 years history of employment with addresses & length of time on job
- One month's recent paystubs & past 2 years W-2 forms
- Verification of other income: social security, child support, retirement, etc.
- If commissioned sales person: past 2 years federal tax returns including all schedules
(Resign & date these documents to attest their authenticity.)
- If self-employed: past 2 years federal tax returns including all schedules and a
year-to-date profit and loss statement (Resign & date these documents to attest their authenticity.)
- Rental income: copy of leases
- Assets:
- Copies of the last three (3) months statements (all pages) showing "Cash Needed for Closing."
- Copies of any financial assets such as retirement accounts and/or mutual funds for last(3) months, to show a
savings pattern. Do not need every account. (Do not make copies of savings bonds)
- Make, model & value of automobiles, boats, etc.
- Creditors: (open accounts & loans)
- Addresses, account numbers, current balance & monthly payment
- Name & address of lender & account number on all presently owned property
- New construction only:
- Plans & specifications
- Contract with builder
- Plot plan
- Divorce Decrees & Separation Agreement (if applicable)
- Name, phone & fax of Title Company and homeowner’s insurance agent
